Program Info

Housing Stabilization Services is a new Minnesota Medical Assistance benefit to help people with disabilities, including mental illness and substance use disorder, and seniors find and keep housing.

Program Eligibility 

Be on Medical Assistance
Be 18-years old or older 
Have a documented disability or disabling condition
Experiencing housing instability ​

Housing Stabilization Services We Offer 

Housing Consultation

We help a person who doesn’t have MA case management develop a person-centered plan that addresses their needs, wants, and goals for living in the community.

Housing Transition

We help a person plan for, find, and move into housing that fits their needs. We will honor and respect person's dignity.  ​

Housing Sustaining 

We help a person keep their housing after they have moved in. Depending whatever the needs are we are here for our client.
